Pest Control Contract
Payment and Services
This Pest Control Contract (hereinafter referred to as the "Contract") is entered into between the Customer (as identified by the name and contact information provided during the payment process) and AVERZION DBA of 95 Applications  (hereinafter referred to as the "Company"), collectively referred to as the "Parties." The Customer agrees to pay the Company a monthly fee of $129 for the pest control services described herein, with no long-term contract required.
Pest Control Services
The Company will provide the following pest control services to the Customer:
a) We administer high-grade pesticides specially designed to target scorpions on a quarterly basis. In certain situations, it may require more frequent applications, and the transition to a quarterly schedule could take an undetermined amount of time.
b) Maintenance of the Slick Barrier coating installed by either the Customer or the Company. The Slick Barrier coating will be warranted as long as the Customer is a member.
c) Reapplication of the Slick Barrier coating if the Customer repaints their home.
d) Reapplication of the Slick Barrier coating if the Customer moves to a new home of similar size.
e) Evening and nighttime pest spraying, allowing the Company to identify the sources of pest infestation.
f) Testing of all pesticides on scorpions to ensure effectiveness.
Scorpion Sightings and Refund Policy
If the Customer sees a live scorpion in their home after the first three (3) months of service, the Company will refund the Customer's $129 monthly fee for that month. The Customer and the Company may terminate this Contract after three (3) or more refunds have been issued within a calander year.
Cancellation Policy
As there is no long-term contract, the Customer may cancel the services at any time without any cancellation fees. However, any payment made for the current month of service will not be refunded.
Governing Law and Jurisdiction
This Contract shall be governed by and construed in accordance with the laws of the Arizona. Any disputes arising out of or in connection with this Contract shall be subject to the exclusive jurisdiction of the courts of the Arizona.
By making the initial payment via the provided link, the Customer acknowledges that they have read, understood, and agreed to the terms and conditions set forth in this Pest Control Contract. The Customer's payment serves as their electronic signature and acceptance of the Contract.
